Medication Bridge Program is a private-public partnership under the leadership of the Foundation for Healthy Communities to help eligible uninsured and under-insured patients of all ages to receive needed medications from pharmaceutical companies' Patient Assistance Programs.  Click here for information on N.H. • Gift of Fitness - Through a partnership with the YMCA, we offer three month complimentary introductory memberships at a patient's local YM/YWCA. Exercise can greatly improve outlook and outcomes for kidney patients and transplant recipients. Referrals come from social workers, nurses and physicians at area dialysis and transplant centers.
