North Dakota - Emergency and Assistance Resources and Programs for Persons with Diabetes National Kidney Foundation of Minnesota, North Dakota and South Dakota - Offers a variety of support services and information. May help assist in locating assistance. Address: 1100 E 21st St Ste 210 Avera Doctors Plaza II, Sioux Falls, SD 57105-1020, Phone: (800) 596-7943 North Dakota Community Action Diabetics Supplies Program A grant form the Dakota Medical Foundation has been received to assist low-income individuals with diabetes medications and supplies. the grant was awarded to the four Community Action Agencies in eastern North Dakota. The program will accept referrals of low-income individuals needing assistance with diabetes supplies. A case management approach will be utilized to assure that the participant will be able to maintain access to needed diabetic supplies. Process. An individual needing assistance will meet with a staff person at DPCAA to complete an application and determin a plan for future needs. A voucher wil be provided which the participant will take to the designated pharmacy to receive approved diabetic supplies.
