The Resource Mothers Project works with at-risk pregnant women, helping them to obtain early prenatal care and other social services. The Resource Mothers act as mentors and are on-call to their clients 24 hours a day for emergencies. They stay involved through the babies' first year to make sure they receive proper immunizations and have a medical home. Direct services include, but are not limited to, education, transportation, and case management.

They also have Bilingual Resource Mothers across the state to serve Spanish-speaking women and their families by translating and helping them overcome other cultural barriers they face.

Services:  Childbirth and parenting classes; Help with scheduling appointments; Help with transportation to appointments; Nutrition counseling; Special Services to substance referrals; WIC and Social service referrals; School programs; and Follow-up phone calls and letters.

Call 1-800-464-4357 or e-mail us at  Delaware Help Line is a toll-free service which provides information on state government agencies and referrals to community resources. It began operation in 1990 as a statewide comprehensive information and referral service for health and human services but broadened its scope considerably in 1997. Delaware Help Line now includes a state government information service that links callers to appropriate state offices, legislators and employees, and continues to provide needs assessment, problem resolution and referrals for callers with health or human service needs.

Application assistance is available for Medicaid, Community Healthcare Access Program (CHAP) and other programs. Services are based on sliding scale fees. Located in Wilmington, Delaware, Nemours Health Clinic provides a variety of services for senior citizens who meet financial qualifications and are citizens of the state of Delaware. These include comprehensive dental care, eye examinations and eyeglasses, and hearing tests and hearing aids. In addition, prescription drugs can be provided to enrollees through the Pharmaceutical Assistance Program at a reduced cost.

The Health Center provides medical care for patients of all ages, including obstetric and newborn care, pediatrics, adult and geriatric care. The Center offers well visits, sick visits, as well as surgery, orthopedic, foot, eye and dental care. It is also one of the state’s largest care providers for HIV patients. The Center has a team dedicated to connecting patients with any needed social services, including financial assistance.

St. Francis Hospital - St. Francis Hospital provides comprehensive healthcare to all individuals. St. Francis Family Practice provides a hospital-based family health practice, including obstetric and newborn care through Tiny Steps, a program for low-income pregnant women, staffed by resident doctors providing health screening, diagnosis, and treatment to patients of all ages. The St. Clare Outreach Program is a mobile van staffed by a doctor, a registered nurse and one assistant that travels to different locations in Wilmington to serve the homeless and others in need. The Center of Hope, a family health center in Newark, offers health care to migrant workers. The Tiny Steps program is also offered at the Newark location.
