California - Emergency and Assistance Resources and Programs for Persons with Diabetes

Dr. Douglas Stephens Patient Scholarship via the National Kidney Foundation of Northern CA - The National Kidney Foundation is proud to assist patients with kidney or renal disease in furthering their educational goals. Applications are made available in May each year and are sent directly to Dialysis and Transplant unit social workers. If you would like one mailed to you, please e-mail your name and address, include "Patient Scholarship Application" in the subject line and send it to eboni@kidneynca.org. Applications for the the 2007-2008 Patient Scholarship are now available. To download the application please click here.

Recipients of the scholarship and the allocation of funds are decided by a dedicated committee of healthcare professionals and members of the renal community. The grants can range from $500 - $1500 and may be used towards tuition, books and other classroom materials. Funds remain available throughout the school year (fall spring) but do not roll over. Students may be awarded the scholarship up to 4 times.

People Like Us, Live! - a series of classes designed to help people with kidney disease (and their loved-ones) understand, treat, and live with their condition.
Patient Emergency Funds - provides monetary assistance to those Kidney Patients in need of temporary assistance with bills and expenses.

RxHelpForCalifornians is a program that connects qualified, low-income people with discount prescription drugs, direct from the pharmaceutical manufacturer.

Children's Diabetes Foundation of the North Bay in San Francisco was founded by Lynn Francisco, homemaker and mother of two. Her daughter was diagnosed with type 1 diabetes in 1994 at age 2 1/2. She organizes get togethers for fellowship and support through her education-based nonprofit organization.

Services: Uses Foothill College Dental Hygiene students to provide services and dental health education to public health clinics, dental vans and other health centers, as part of their educational program. Services will assist low-income and underserved individuals so that they may improve and take charge of their dental health.

Services: The Supply Bank provides low-income, uninsured or underinsured individuals access to free glucose meters, testing strips, lancet devices, and lancets for up to three months. Participation in the Supply Bank will also afford clients with the opportunity to access the diabetes self-management education classes and referrals to low-cost healthcare services.

Services: Diabetes Health Education Program for Seniors, which provides health educators, who are experienced nurses and dieticians, that travel to various senior centers to provide introduction and blood-glucose screenings. Those clients diagnosed with diabetes will then participate in seven hours of classroom instruction about diabetes and the successful management of the disease.

Kids with Diabetes is a California, nonprofit organization that provides charitable assistance to families with diabetic children. It also dispenses information to educate the general public regarding the diagnosis, treatment, and management of type 1 diabetes. Its programs include the Fun for Type 1 Playgroup, a monthly support group for kids and families with diabetes. At each meeting, there are games, fun activities, and informative guest speakers. The New Kids Program provides a helping hand during the critical time immediately after diagnosis. The Teen Mentors Program provides the teenager with diabetes help during this difficult age. And the Sharps Collection Program provide a safe, easy way to properly dispose of full sharps containers.
